To be eligible for housing, applicants must have a qualifying behavioral health diagnosis and be receiving behavioral health services. The application packet includes a letter that explains each step, including how to complete the Treatment & Diagnosis Verification Form with your provider.
Once you’ve completed the forms, you can return them in the way that works best for you. After we receive your paperwork, our team will review everything and contact you if anything else is needed.
If you’re unable to print the application, please call us. We can mail a copy to you, or you may pick one up at our office Monday through Friday, 8 AM to 4 PM.
